GOLDEN VALLEY HUMAN SERVICES FOUNDATION SUB-COMMITTEE MINUTES OF SEPTEMBER 28, 1992
Meeting held at the Golden Valley City Hall, Lower Level #2 Conference Room.
Members Present: Don Anderson, Chair; Ed Hara;
The meeting was called to order at 7:00 pm.
The committee discussed the purpose of the sub-committee meeting which was to
create a questionaire for recipients 0r the Foundation.
The chair proposed that the planning for the Foundation be divided into six areas.
I. Soliciting interested agencies/services in the Golden Valley area.
In this area the committee discussed a "Letter of Interest" to be sent to
proposed recipients to be filled out and returned requesting the "Application
Package" from the Foundation be sent back to the recipients to complete.
A proposed "Letter of Interest" is attached to these minutes.
II. "Application Package" for recipients to use in applying for Foundation funding.
The package should contain:
A. Foundation requirements for eligible recipients information.
B. Foundation Application for Funding.
C. Required documents to be submitted with Application for Funding.
D. Timetable schedule setting forth the date. the Application for Furlding
together with the other documentation needs to be received at City Hall,
the date the Foundation will review the Applications for Funding,
and the date the Foundation will make it's recommendation to the Golden
Valley City Council.
t. The mission statement of the Golden Valley Foundation as approved by the
Golden Valley City Council.
Items A, Band C are recommended as attached to these minutes. Item D needs
to be discussed by the Foundation and staff liaison. Item E is available in
the City Council Ordinance creating the Foundation.
III. Review of the "Application Packages" received from potential. recipients.
A standardized review summary form needs to be created for the use of the
Foundation when examining the recipient's "Application Package". The sub-
committee did not discuss this form as the most pressing need was the
"Applicatic:hn Package".
IV. Recommendation of potential recipients to the City Council.
V. An assessment evaluation followup of the recipient':s impact on their respective
problem/services and the recipient's impact on the City of Golden Valley.
VI. Internal Foundation recommendations for future funding and estimates for
available monies to the Foundation.
Items III., IV, V~~ and VI were not discussed in depth by the committee.
The meeting was adjourned at 8:15 pm.
