order at 1:34 pm, in the City of Golden Valley Council Conference Room.

1. Kloepper repo t Xcel Energy Is close to finishing the low voltage repair necessary
so that Crystal's large pumps will not shut down during extreme hot weather
temperatures. Crystal is not using its large pumps until the work is done.
2. The Minneapolis Water Advisory Board meeting is Thursday, August 23, Golden Valley
Mayor Harris may be unable to attend. Clancy will ask if the Golden Valley's Mayor -Pro
Tem Mike Freiberg will attend in his absence.

3. The city councils for the cities of Golden Valley and New Hope approved the resolution
authorizing the DeCola Ponds and Medicine Lake Road Area Feasibility Study. The City
of Crystal did not approve the resolution. In the past, the Bassett Creek Watershed
Management Commission has taken on the role of mediator to resolve similar situations
and Golden Valley is considering that approach.

At the August 1, 2012 Joint Water Commission (JWC) meeting, the leak detection contract with
Water Conservation Service, Inc. was approved.
The joint water system leak detection survey found 15 total leaks in the three cities. No leaks
were found on the mains owned by JWC (greater than 12 inches). Leak detection results per city
are listed below:

Minneapolis/Golden Valley Water Meter Improvements
The JWC Water Meter Replacement Project (12-10) is currently out for bid. The total project
estimated cost is $720,000. Following the bid opening, a breakdown of the applicable costs will
be provided to both the Joint Water Commission (JWC) and City of Minneapolis for approval. The
following is a schedule of key project milestones that will occur between now and the time of
construction on the metering facility.
Key Dates:
Tuesday, September 18, 2012: Bid opening will be held at Golden Valley City Hall at 9 a.m.
Wednesday, October 3, 2012: Bid information will be provided in the JWC agenda packet for
review and approval. The JWC needs to approve the budget amount prior to award of contract.
Wednesday, October 3, 2012: Bid information will be provided to the City of Minneapolis for
approval prior to the award of the contract.
Tuesday, October 16, 2012: With approval from the JWC and City of Minneapolis, the City of
Golden Valley will award City Project (12-10) at the City Council meeting.

EMERGENCY PUBLIC WATER SUPPLY, HENNEPIN COUNTY
The Department of Natural Resources has received notice from the Minnesota Department of
Health of the reactivation of the New Hope Well #1 unique number 203542) as an emergency
water source for the City of New Hope. A review o the State Database indicated that the City of
New Hope does not have a DNR Appropriation Permit for an emergency supply system. Please
be advised that a water appropriation permit will be needed from the Department of Natural
Resources if the City of New Hope believes that more than 10,000 gallons per day, or one
million gallons per year, will be pumped from Well #1 when an emergency water source is
required for the City of New Hope. Should the City of believe that well #1 will exceed the above
thresholds, then the City of New Hope should apply for a DNR Appropriation Permit by
submitting the completed apppplication form (attached) with supportingg information to Area
Hydrolog�tst, Kate Drewry, T3NR Ecolog�'cal and Water Resources, 1 Z00 Warner Road, Saint
Paul, MN 55106. Please note that the DNR Appropriation Permit will need to be obtained before
well #1 can be used, and that additional information may be requested should any issues arise.
Please note specific requirements related to conservation and the statement of justification.
Minnesota Statutes and rules require all water users to employ the best available means and
practices to promote the efficient use of water. We are encouraging public water suppliers to
implement conservation measures and programs to reduce water demand. Actions taken to
reduce demand can negate or delay the need to develop additional water supplies, save money
and conserve water resources for future use. During review of amendment requests, the
Department will evaluate alternatives to the proposed well including the effectiveness of
conservation measures.

Note: Temporary appropriations are one-time projects that do not continue from year to year. Temporary permits are Issued for
appropriations with durations of up to 12 months. Time extensions may be requested, but the total length of time the permit remains In force
cannot exceed two years.
8. SOURCE OF WATER: Mark only one box.
Note: Submit ane application for each source of water or system. For example, several wells in the same aquifer manffolded into
one system constitute one source; however, a stream and a gravel pit are two sources of water and would require separate applications.

Note: If your well(s) is located in an aquifer for which hydrologic data are limited or unavailable, you may request to submit data
regarding area wells.
c. STREAM, DITCH, or RNER - Identity it and submit a contingency plan describing the alternatives you would utilize if the appro-
priation is restricted because of low water conditions. If no alternative water supply is available, you must submit a written statement agreeing
to withstand the results of no appropriation.
Note: Only temporary appropriations from designated trout streams may be approved.
WETLAND, LAKE or IMPOUNDMENT - identify it and submit the following:
1) A contingency plan (see 8.c.).
2) For basins less than 500 acres in size you must:
a) Notify all riparian landowners and submit a list of those landowners.
b) Obtain a signed statement from as many of those riparian landowners as possible which states their support of the
proposed appropriation.
c) Provide an accounting of the number of signatures of riparian owners you are unable to obtain.

10. MEANS OF TAKING AND RATE: If you mark "d", specify the method to be used and the rate of taking (in gallons per minute or cubic feet
per second). If multiple wells or pump sites are to be used, attach a letter of explanation (see #9).
11. METHOD OF MEASUREMENT: Fill in as appropriate.
Note: Flow meters are required by Minnesota Statutes 103G.281, Subdivision 2, for measuring the quantity of water appropriated
within the degree of accuracy required by rule (10%). The DNR can approve other methods of measurement. Timing devices, including hour meters
and electric meters, are approved devices if there is a constant rate of appropriation. To obtain approval for other methods of measurement,
applicants must submit a written request with the application that includes a description of the proposed method.
Also of Note: All permit holders are required to measure and keep monthly and yearly records of the quantity of water
used or appropriated.
12. MEANS OF CONVEYANCE: FIA in as appropriate.
13. LEGAL DESCRIPTION: Describe the property that will be affected by your project (example: T101N, R14W, Section 5, NWIA and
N112 SWIM). If property other than what you own will be affected, you must attach a copy of the land agreement which
Includes a) the legal description of the property, b) the termination date of the agreement, and c) the signature of all parties.
14. MONTHS OF APPROPRIATION: Fill in as appropriate.
13. SCHEDULE OF APPROPRIATION: Mark only one box to Indicate your schedule. For temporary projects, the appropriate years) should

16. TOTAL ANNUAL USE: (In million of gallons per year).
Note: Appropriation in excess of 2 million gallons/day may need legislative approval (See Minnesota Statutes 103G.265,
Subdivision 3, for exemptions): appropriation averaging 30 million gallons/month for commercial or industrial purposes may require the
preparation of an Environmental Assessment Worksheet (see Minnesota Rules 4410.4300, Subpart 24).
17-19. Complete if applicable. In Question 17, indicate the quantity of water to be discharged.
20. ADDITIONAL REQUIREMENTS: Submit the following as part of your application. All applications must include:
a. Map or air photo showing the project site.
b. Describe alternative sources of water and methods, Including conservation practices that were considered and why the proposed
aitemative was selected.
c. Additional documents, letters, or statements required.
21. APPLICATION FEE: A minimum application fee of $150 is required for each permit application. An application fee of $300 Is required
for after -the -fact permit applications. Water use reporting and fees for years in which water appropriations occurred without a permit will also apply.
Please do not submit fees with the permit application, you will be billed separately. Permits cannot be issued until all fees have been paid.
MAILING: Submit the following to the appropriate DNR Area office (see map on back page for addresses):
1) application (keep a copy for your records).
2) supporting documents.
3) do not send application fee with the application. You will be billed separately.
Make sure that you furnish all Information that is requested. Forts that are incorrectly filled out or lack requested Information will cause a delay in
your application.
LOCAL REVIEW: Minnesota Statutes allow local units of government 30 days to review your project and submit comments to the DNR.
A copy of your application will be submitted by the DNR to:
1) local soil & water conservation district
2) watershed district
3) city
ADDITIONAL DATA: You may be required to submit additional information regarding your project. You will be notified if this information is required.

SCOPE: This report covers the design of this project insofar as the safety and sanitary quality of the water for
public consumption may be affected, and applies to this project only and not to the entire system or any other part
thereof. The examination of plans is based upon the supposition that the data on which the design is based are
correct, and that necessary legal authority has been obtained to construct the project. The responsibility for the
design of structural features and the efficiency of equipment must be taken by the project designer. Approval is
contingent upon satisfactory disposition of any requirements included with this report. It is the project owner's
responsibility to retain the plans at the project location.
Water supply plans are examined with regard to the location, construction and operational features of the design
and maintenance of all parts of the system which may affect the safety and sanitary quality of the water.
Examination is based on the standards of this department.
It is the contractor's responsibility to contact the Minnesota Department of Health (MDH) for inspections as

This project proposal consists of the reactivation of Well No. 1 (Unique No. 203542) as an Emergency Backup
Community Water Supply Well. Well No. 1 is a 24 -inch by 16 -inch steel cased well, approximately 342 feet,
finished in the Jordan formation. The 24 -inch casing extends to 205 feet, and the 16 -inch casing to 315 feet, and
there is 22 feet of open hole. The legal location of the well is the NW '/4 of SW '/, of SE %a of Section No. 7,
TI 18N, R 21 W, Hennepin County.
Information submitted with the proposal and records on file at the MDH indicate that the well was completed by
Bergerson-Caswell, Inc., in June, 1959, using solid tool (cable too[) drilling method. The well was completed as a
24 -inch by 16 -inch diameter, 422 -feet bedrock well with open hole in the Prairie du Chien and Jordan formations.
Construction consisted of driving the 24 -inch diameter steel casing into the top of the Prairie du Chien Group to
213 fat, drilling a 24 -inch diameter bore hole to 422 feet, installing 16 -inch diameter steel casing to 314 feet, and
grouting the annular space between the casings with cement from 245 feet up to the ground surface. The casing
penetrates unconsolidated quaternary deposits from the ground surface to 181 feet, St. Peter sandstone from 181 to
202 feet, Prairie du Chien Group from 202 to 339 feet, and Jordan sandstone from 339 to 422 feet. The
December 14, 2011 down hole video showed that the open hole had been filled in to a depth of 334 feet.

Requirement(s):
Well
1. The MDH review of the December 14, 2011, down hole video did not reveal any obvious defects that would
rule out use of this well for a community water supply. The video showed evidence of minor corrosion and/or
encrustation on the casing, but there were no visible holes, cracks or fractures in the casing. The annular space
around most of the bottom of the 16 -inch casing appeared to be open, supporting previous reports that the
annular space surrounding the 16 -inch casing is not grouted below 245 feet. A piece of cable is partly
cemented into the side of the bore hole below the casing. A small diameter pipe extends vertically several
inches above the fill in the bottom of the well.
2. The pumphouse may be used only for the containment and protection of the well, pump, water treatment
equipment, and water treatment chemicals, if any. No other use of the building is permitted. The building
must not be used for vehicle or other equipment storage or maintenance, and must not be used for storage or
mixing of fuel, paint or other chemicals.
3. The well must be accessible to a drilling machine or hoist for maintenance, repair and sealing of the well. The
pumphouse must be provided with a roof hatch or comparable access to the well.
4. The well location must comply with the contaminant setback distances specified in Minnesota Rules,
part 4725.4450, but in no case be less than 50 feet, from a source of contamination, except as specified in
Minnesota Rules, part 4725.5850, subpart 2. No storm sewer or buried sanitary sewer, including floor drains
and other buried sewer service lines under a building, may be located less than 50 feet from the well. Any
existing storm or sanitary sewer located less than 50 feet from the well must be permanently disconnected
from service, and the sewer pipe removed or the ends permanently plugged with concrete such that sewage
cannot come within 50 feet of the well.
S. The well must have an MHD approved buried pitless connection, or an above -grade discharge in a
pumphouse.
6. Modifications to the well, including any changes to the wellhead, must not be made until after plans have been
submitted to and approved by the MDH.
7. All well contracting activities, including wellhead completion, pump installation, and discharge head
connection must be done by or under the direct supervision of a person licensed by the MDH to perform those
activities.
8. The well owner must obtain a Water Use Permit in accordance with the requirements of the Minnesota
Department of Natural Resources.
9. The city of New Hope must contact the following offices if the emergency water supply well is ever placed
into production: The MDH, city of Minneapolis, city of Crystal, and the city of Golden Valley. In addition,
the city of New Hope must provide a copy of the action plan to the MDH when the well is placed into
production. This plan should be submitted to our office as soon as possible.

10. Since the laboratory reports indicate that the combined radium 226 + 228 is 5 picocuries per liter (pCi/L) (see
laboratory report), the city of New Hope must contact the MDH district engineer, Isaac Bradlich, at 651/201
3971, if the well is ever placed into production.
11. Please verify that only clear water wastes are allowed to discharge into the floor drain inside the pumphouse.
If anything other than clear water wastes are going to discharge to the floor drain, then another approved
method of sewage disposal must be provided.
12. Prior to permanent sealing of the well, the small diameter pipe currently at a depth of 334 feet and any other
obstructions that may interfere with sealing must be removed from the well; the fill material must be removed
to within 10 feet of the original bottom of the well; and the 16 -inch casing must be ripped or perforated from
the bottom of the casing up to a depth of 245 feet, in accordance with Minnesota Rules, part 4725.3850,
subpart 3.
Well Site Inspection
13. The site location was inspected on November 1, 2011, by Amal Djerrari of the MDH. Mr. Guy Johnson and
Mr. Bernie Weber, from the city of New Hope, and Mr. Steve Nelson, from Bolton and Menk, Inc., were
present to answer questions about the setback distances from potential contamination sources and the locations
of property boundaries. The well is already constructed. This is a disaffected well that the city wants to
rehabilitate and use as an emergency well.
Information provided to the MDH indicates that the city of New Hope owns the parcel on which the well is
located. The parcel boundary, however, is less than fifty feet from the well. The city has entered into an
easement agreement with the school district for permanent control of the property needed to control the area
within 50 feet of the well site. A copy of the easement is contained in the Emergency Well Reactivation
Submittal.
The well is completed in the Prairie du Chien -Jordan Aquifer, which has sufficient geologic cover over it to
retard the vertical movement of contamination. The original well was completed in the Jordan Aquifer to a
depth of 422 feet. The contact between the Prairie du Chien and the Jordan is at 339 feet. The hole is filled
with caving material from 342 feet to the bottom. The driller tried to bail to 422 feet, but the hole kept
refilling to 342 feet.
The floor drains are connected to a pipe that exits the building on the east side, heads south, and then west
towards the storm or sanitary sewer mains on Aquilla Avenue. The city tried to verify the potential connection
to the sewer but could not confirm it. They are thinking of plugging the known portion of the pipe that exits
from the building. Except for this issue, all setback distances appear to be met and other siting criteria
specified in Minnesota Rules, chapter 4720, based on the location of the well and our evaluation of the
surrounding area.
Note(s):

old or existing water tower.

COMPLIANCE: No construction shall take place except in accordance with the approved plans. If it is desired to
make deviations from the approved plans, the MIH should be consulted and approval of the changes ohtained
before the construction is started; otherwise, such construction is installed without proper approval and, in addition,
may create dangers to public health.
Authorization for construction in accordance with the approved plans may be withdrawn if construction is not
undertaken within a period of two years. The fact that the plans have been approved does not necessarily mean that
recommendations or requirements for change will not be made at some later time when changed conditions,
additional information, or advanced knowledge make improvements necessary.
